- Assisted/supportive living referrals
- Referrals to home health care agencies
- Referrals to equipment companies
- Referrals to alternative for the older adult/Rehabilitation Department
- Elder and child abuse referrals
- Completion of Living Wills, Health Care Power of Attorneys, and wallet cards

- Emergency Department visits
- O.B. routine referrals and visits
- Intensive Care Unit visits
- Referrals for outpatient psychiatric assistance
- Referrals for prescription assistance
- Financial evaluations for assistance
